Children's Cancer Association ("CCA") respects the intellectual property rights of others and expects you as a user of its Website (www.mymusicrx.org) to do the same. If you believe that your copyrighted work has been infringed in any respect on this Website, you may notify CCA's copyright agent as set forth below ("Notification").

Your copyright infringement Notification must comply with the Digital Millennium Copyright Act ("DMCA") in order to be effective. Please review 17 U.S.C. Sec. 512(c)(3) of the DMCA (http://www.copyright.gov/title17/92chap5.html) or consult with an attorney prior to sending a Notification. Please be advised that the information in your Notification may be provided to the person responsible for the allegedly infringing material.

To be, a Notification of claimed infringement must be a written communication provided to CCA's designated copyright agent that includes substantially the following:

(1) A physical or electronic signature of a person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed; and

(2) Identification of the copyrighted work claimed to have been infringed, or, if multiple copyrighted works at a single online site are covered by a single Notification, a representative list of such works at that site; and

(3) Identification of the material that is claimed to be infringing or to be the subject of infringing activity and that is to be removed or access to which is to be disabled, and information reasonably sufficient to permit CCA to locate the material; and

(4) Information reasonably sufficient to permit CCA to contact you, the complaining party, such as an address, telephone number, and, if available, an electronic mail address at which you may be contacted; and

(5) A statement that you have a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law; and

(6) A statement that the information in the Notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
